No paper towels or mechanical hand drying device provided at handwash sink. There are no paper towels at the hand wash sink in the kitchen. Operator provided paper towels.

Spray bottle containing toxic substance not labeled. There us a unlabeled chemical spray bottle at the front counter. Operator labeled the chemical spray bottle.

Use of cooking equipment producing grease laden vapors/smoke with no hood suppression system installed. Notified Fire AHJ. For reporting purposes only. Operator is frying eggs and bacon on a table top flat top grill with no hood or fire suppression system.
